All employees must either enroll or waive coverage for Medical, Dental and Vision by accessing the Benefits Module through Ceridian DayForce.
- User Name: Your Employee ID Number as 6 digits using leading zeros (i.e., 4956 becomes 004956)
- Password: Your date of birth as MMDDYYYY

Enrollment Instructions:
- Navigate to Benefits Section. Log in to Dayforce and enter BENEFITS section by clicking the BENEFITS icon
- Launching the Enrollment Process. To begin the enrollment process, select OVERVIEW tab. Available enrollments will be listed under the ENROLLMENTS section. To begin the enrollment process, click START ENROLLMENT.
- Viewing Eligible Benefits. You will be directed to the INTRODUCTION page. Benefits that you are currently eligible for will be listed in the right hand column. Click NEXT in the upper right corner to continue enrollment process.
- Add/Edit Beneficiaries and Dependents. In the PROFILE section, add and/or edit beneficiary and dependent information.
- After all dependents/beneficiaries have been added/edited, click NEXT to continue.
- Please Note: Beneficiary information input at this step will be used throughout the remainder of the enrollment process. All fields marked with an asterisk are required and must be completed.
- Selecting Benefit Coverage. In the ELECTION section, view all eligible benefit coverage and tiers. If dependent information was not completed in PROFILE section, you will only be eligible for EMPLOYEE ONLY coverage.
- Selecting a benefit option will open a window with coverage details including:
- Employee Cost per pay period (lower number in blue)
- Company Cost per pay period (top number in black)
- Ability to add dependents for that particular benefit option
- Click on REFERENCE ATTACHMENTS to view the following:
- Additional detailed plan documentation
- Required documentation for dependent coverage
- Please Note: Documentation is not required for dependents who are currently under benefit coverage.
- Employees eligible for Life and AD&D and Allstate Plans: Your beneficiary must be added in the OPTION DETAILS window.
- Selecting a benefit option will open a window with coverage details including:

Be sure to have the Social Security numbers and birth dates for any eligible dependent(s) that you plan to enroll. You cannot enroll your dependent(s) without this information. If you add a new dependent and elect coverage for them, applicable documentation must be submitted.

Employees
All full-time employees regularly working at least 30 hours per week are eligible to enroll themselves and their eligible dependents in medical, dental, and vision coverage following the waiting period.
- For exempt employees, you are eligible to enroll in life and disability coverage following 90 days of continuous employment.
- For non-exempt employees, you are eligible to enroll in life and disability coverage following one year from date of hire of continuous employment.
In order to continue eligibility throughout the year, you must regularly maintain at least 30 eligible hours per week.
Family Members
If you are an eligible employee and elect coverage, you can also elect the following coverage for your eligible family members:
- Medical
- Dental
- Vision
- Flexible Spending Account
- Dependent Care Spending Account
- Life & AD&D
- Accident
- Critical Illness
- Hospital Indemnity
- Legal
You must have coverage for yourself to enroll your eligible family members.
Your Eligible Family Members Include:
- Your legal spouse
- Your dependent children up to age 26 — including biological children, adopted children, stepchildren, or those for whom you have legal custody or guardianship by court decree
- A dependent child currently covered on your policy who is disabled mentally or physically, as defined by the Social Security Administration may continue on your policy

You can change your benefit elections during the plan year if you have a qualifying life status event as defined by the IRS.
Life Status Events Include:
- Marriage or divorce
- Birth, death, or legal adoption
- Associate gains or loses coverage
- Family member gains or loses coverage
Documentation for all qualifying life events must be submitted via Dayforce (dayforcehcm.com) within 30 days of the event. Life event coverage will begin on the effective date of the covered event once documentation has been approved.
